Are you interested in finding out more about the Practice? Would you like to influence the development of local health services?
We have set up a Patient Reference Group and a Virtual Patient Group with the aim of improving two way communication.
We won’t ask you any medical questions, only general questions about the practice. For the Virtual Patient Group, this will usually be in the form of a questionnaire.
This is a volunteer group of patients which held its first meeting on 16th November 2011. They meet regularly (perhaps every two months or quarterly) to discuss patients’ needs, priorities and issues to enable patients to contribute to the improvement of services and support the Practice to implement changes. The group come from a variety of backgrounds within the local community, but have one thing in common – they are all patients of Abbey House Medical Practice!
If you would like to meet other patients and have your say in influencing decisions, then please download and complete the Patient Participation form below and return to the Surgery. Alternatively, please e-mail the Chairman on firstname.lastname@example.org for more details.
This is a volunteer list of patients with whom we communicate via e-mail. The Practice will contact members of this group to ask them questions about the surgery, how well we are doing and ensure changes that are being made are patient focused.
We will try not to contact you too often and you do not need to respond every time, but obviously if we have more views then we have a better chance of getting it right!
If you would like to have your say, but don’t have the time to attend a face to face meeting, then please download and complete the form below and return to the Surgery.
